Every year, roughly 600 million people worldwide fall ill from eating contaminated food, and over 420,000 of them die. These are not just statistics – they represent a massive, preventable public health crisis. At the heart of preventing these illnesses are two interconnected practices: hygiene and sanitation. Whether food is being grown on a farm, processed in a factory, or prepared in a kitchen, the cleanliness of people, surfaces, and environments determines whether that food reaches your plate safely or carries invisible dangers.

What hygiene and sanitation mean in food safety
In food safety, hygiene refers to the conditions and practices that prevent contamination at every stage of the food chain. It covers the cleanliness of the food itself, the health and habits of food handlers, and the sanitary state of the facilities where food is prepared. Sanitation, on the other hand, refers specifically to the cleaning and disinfection procedures applied to equipment, surfaces, and environments to eliminate harmful microorganisms.
While the two terms are sometimes used interchangeably, they play distinct roles. According to the World Health Organization (WHO), unsafe food containing bacteria, viruses, parasites, or chemical substances causes more than 200 different diseases. Hygiene and sanitation together act as the primary line of defence against these hazards – from the farm all the way to the consumer’s table.
The global burden of poor food hygiene
The scale of foodborne illness caused by inadequate hygiene is staggering. WHO estimates indicate that nearly 1 in 10 people around the world fall ill each year after eating contaminated food, with children under five being disproportionately affected – they account for around 125,000 deaths annually from foodborne diseases. Low- and middle-income countries bear the heaviest burden, losing an estimated US $110 billion each year in productivity losses and medical costs due to unsafe food.
These numbers underline a critical point: foodborne diseases are not just a personal inconvenience. They strain healthcare systems, damage economies, and disproportionately harm the most vulnerable populations – young children, the elderly, pregnant women, and immunocompromised individuals.
Personal hygiene of food handlers
People who handle food are one of the most common sources of contamination. A single lapse – an unwashed hand, a sneeze over an open container – can introduce pathogens like Salmonella, E. coli, or Norovirus into food. That is why personal hygiene of food handlers is considered a non-negotiable foundation of food safety.
Handwashing
Handwashing is the single most effective practice for preventing the spread of foodborne pathogens. The U.S. Centers for Disease Control and Prevention (CDC) recommends washing hands with soap and water for at least 20 seconds – before, during, and after preparing food. Food handlers must wash their hands after touching raw meat, using the restroom, handling waste, or touching their face or hair. Handwashing stations should be easily accessible and well-stocked with soap and disposable towels.
Health and illness reporting
Food handlers who are experiencing symptoms such as vomiting, diarrhoea, or fever must not prepare or serve food. Pathogens like Hepatitis A and Norovirus can spread rapidly through food prepared by sick individuals. Most food safety guidelines require that ill employees stay away from food preparation areas until at least 24 hours after symptoms stop.
Clothing and appearance
Clean work uniforms, hair restraints (including beard nets), and the removal of jewellery before handling food are all standard requirements. Dirty clothing can carry bacteria even without direct contact with food. Dedicated work attire that is changed daily helps reduce this risk significantly.
Cleaning versus sanitizing: understanding the difference
One of the most common misconceptions in food safety is that cleaning and sanitizing are the same thing. They are not. Cleaning removes visible dirt, grease, and food residue from a surface. Sanitizing reduces the number of harmful microorganisms on that surface to safe levels. Both steps are necessary, and they must happen in sequence – a surface that has not been properly cleaned first cannot be effectively sanitized, because food residue shields bacteria from sanitizing agents.
Food-contact surfaces such as cutting boards, countertops, knives, and processing equipment must be both cleaned and sanitized. Non-food-contact surfaces like floors and walls need regular cleaning, but sanitizing is essential for any surface that touches food directly.
The five-step sanitation process
Effective sanitation in food facilities typically follows a systematic five-step approach. First, dry cleaning removes loose debris and food particles. Second, wet cleaning with detergent and warm water loosens remaining residue. Third, rinsing washes away detergent and loosened contaminants. Fourth, sanitizing with an approved chemical agent or hot water kills remaining pathogens. Fifth, air drying prevents recontamination from towels or cloths. Common sanitizers used in food facilities include chlorine-based solutions, quaternary ammonium compounds, and hot water maintained at or above 77°C (171°F).
Preventing cross-contamination
Cross-contamination occurs when harmful microorganisms are transferred from one food item, surface, or person to another. It is one of the leading causes of foodborne illness outbreaks. Raw meat, poultry, seafood, and eggs are the most common sources of cross-contamination in kitchens and food processing units.
Preventing cross-contamination requires separating raw and ready-to-eat foods at every stage – during storage, preparation, and serving. Dedicated cutting boards, utensils, and containers should be used for raw products. Colour-coded kitchen tools help staff quickly identify which items are designated for raw versus cooked foods. Proper cleaning and sanitizing of surfaces between tasks is equally important.
Temperature control and the danger zone
Bacteria multiply most rapidly in what food safety experts call the danger zone – the temperature range between 4°C and 60°C (40°F to 140°F). Food left in this range for extended periods becomes a breeding ground for pathogens. Perishable food should not stay at room temperature for more than two hours (or one hour if the ambient temperature is above 32°C/90°F).
Refrigerators must be maintained at or below 4°C, and hot-held foods should remain at 60°C or above. Using calibrated food thermometers is the only reliable way to confirm that cooking temperatures are met. For example, the CDC recommends cooking poultry to an internal temperature of 74°C (165°F), ground meats to 71°C (160°F), and whole cuts of beef or pork to 63°C (145°F) with a three-minute rest.
Sanitation in food processing and manufacturing
In food processing facilities, sanitation goes well beyond wiping down a countertop. It involves documented procedures covering the cleaning and sanitizing of every piece of equipment, every surface, and every area of the facility. In the United States, the FDA requires food businesses to maintain Sanitation Standard Operating Procedures (SSOPs) – written procedures that outline how contamination and product adulteration are prevented. These SSOPs must be validated, reviewed, and consistently implemented.
Globally recognised food safety certification programmes such as SQF, BRCGS, and FSSC 22000 require advanced sanitation systems, including documented cleaning procedures, risk assessments, employee training, and environmental testing to verify cleaning effectiveness. Facilities must also implement effective pest control measures, maintain adequate ventilation and lighting, and ensure that only potable water is used in food processing and cleaning.
Role of HACCP in maintaining hygiene standards
The Hazard Analysis and Critical Control Points (HACCP) system provides a structured, preventive approach to food safety. Rather than relying on end-product testing alone, HACCP identifies potential hazards at each stage of food production and establishes critical control points (CCPs) where those hazards can be prevented, eliminated, or reduced to safe levels.
Hygiene and sanitation are foundational prerequisites for any HACCP plan. Without proper cleaning schedules, personal hygiene protocols, and facility maintenance, the entire HACCP system loses its effectiveness. Good Hygienic Practices (GHP) and Good Manufacturing Practices (GMP) form the base on which HACCP is built – a point emphasised by international food safety bodies including the WHO and the Codex Alimentarius Commission.
Food hygiene regulations in India: the role of FSSAI
In India, food hygiene and sanitation are regulated by the Food Safety and Standards Authority of India (FSSAI) under the Food Safety and Standards Act, 2006. FSSAI sets science-based standards for food products and enforces hygiene requirements across the food supply chain – from manufacturing and storage to distribution and retail.
Schedule 4 of the FSSAI regulations lays out detailed hygiene and sanitation requirements for food businesses of all sizes. These include maintaining clean premises (floors, walls, drains, and work surfaces), using potable water for food preparation and cleaning, ensuring adequate ventilation and lighting, implementing pest control measures, and enforcing strict personal hygiene among food handlers. Food businesses must also maintain documentation of all sanitation activities.
FSSAI also runs training initiatives such as the FoSTaC (Food Safety Training and Certification) programme, which trains food handlers and supervisors on contamination threats, sanitation standards, and operational controls. Non-compliance with FSSAI hygiene regulations can lead to financial penalties, product seizures, or cancellation of the food business licence.
Hygiene practices for consumers at home
Food safety is not only the responsibility of food businesses. A significant number of foodborne illnesses originate in home kitchens, where basic hygiene practices are often overlooked. The WHO’s Five Keys to Safer Food offers practical guidance for consumers: keep clean, separate raw and cooked foods, cook thoroughly, keep food at safe temperatures, and use safe water and raw materials.
Simple steps make a big difference – washing hands before and after handling food, keeping kitchen surfaces clean, storing raw meat separately from other foods, refrigerating leftovers promptly, and checking that food is cooked to the correct internal temperature. These practices are especially important for households with young children, elderly members, or anyone with a weakened immune system.
The economic and social case for food hygiene
Investing in hygiene and sanitation is not just a moral obligation – it makes strong economic sense. Foodborne illness outbreaks damage brand reputations, lead to costly product recalls, and result in legal liabilities. For food businesses, maintaining high hygiene standards builds customer trust and loyalty. For governments, robust food safety systems protect public health, reduce healthcare costs, and support international trade.
According to the Journal of Food Protection, technological advances in sanitation – including automation, IoT-based monitoring, and sustainable cleaning technologies – are increasingly being integrated into food manufacturing to improve hygiene outcomes and operational efficiency. These innovations are helping businesses move from reactive cleaning to proactive, data-driven sanitation management.
Building a culture of food hygiene
Rules and checklists are essential, but lasting food safety requires a culture of hygiene – an environment where every person, from the CEO to the newest kitchen worker, understands and values clean practices. This begins with leadership commitment: when management visibly prioritises hygiene, staff follow. Regular training, clear communication of expectations, and systems that make reporting concerns easy all contribute to a strong hygiene culture.
Monitoring and accountability are equally important. Regular inspections, hand hygiene audits, and sanitizer concentration checks demonstrate that standards are being upheld – not just written down. When hygiene becomes part of an organisation’s identity, compliance improves, incidents decrease, and food safety becomes second nature rather than a burden.
